Sinai Urban Health Institute works in partnership with community members and organizations to document disparities and improve health in some of the most vulnerable neighborhoods in Chicago.
The Sinai Community Institute utilizes intensive case management to identify and eliminate barriers that impact the social well-being and health status of individuals, families, and the community.
